UIPA Requests

To submit a request under the Uniform Information Practices Act (UIPA), complete the State of Hawai‘i Office of Information Practices’ request form. Your request must include sufficient contact information for the agency to correspond with you, reasonably describe the record you are requesting and state how you would like to receive it, such as by email, mail or in-person pickup.
